Continuity mistake: In nearly every episode, the dark blue jackets that Gage and DeSoto wear suddenly appear and/or vanish and then reappear, numerous times, while en route in their squad, or when they pull into the stationhouse and Rampart. Since stock footage is usually used when showing all the guys getting into their vehicles, Johnny's hair length may differ as well.

Revealing mistake: When Engine 51 and/or Squad 51 are dispatched, the shots of the vehicles when they are first dispatched, while en route to the call or en route to Rampart, etc, in many instances, are stock footage or repeat shots which show the same traffic, same parked cars, same people, etc., from previous episodes.

Continuity mistake: The worn areas of Jack's leather hat drastically changes throughout the film. Additionally, the hat has a threaded 'X' on both its right and left side. In numerous close-ups extra thread holes are visible in different areas near each of these Xs; such as in the Locker, when Jack lies on the ground as the Pearl passes by and later, on the little island, before Jack switches places with Will. In other shots those extra holes are gone, such as when Jack sets sail for the Fountain of Youth. The actual sizes of the Xs also change.

Continuity mistake: When Beckett boards the Dutchman and speaks to Jones, he has very noticeable dark facial stubble, but bizarrely, in his next close-up he is actually clean shaven, his wig curl has shrunk and his eyebrow hair is neater.

Continuity mistake: Aboard the Pearl, after Will orders the rum to be loaded, and Gibbs responds, "Aye, the rum too," the cloth round Gibbs's neck has the loop to his right, but in previous and following shots it's actually "tied" round his neck the opposite way, with the loop to his left. Continuity mistake: When Castor (as Sean Archer) is speared, as he stands there singing morosely his shirt is drenched in blood, but when he is next seen in the ambulance that blood on his shirt has vanished. Additionally, the cuts on his face produce blood trails which smear, when he is first speared, but in the ambulance his face is cleaned up and the blood trails are thin and neat.

Plot hole: In a close-up, just as Jack is freed from the hole in the water wheel, we see the axle's circular wood frame with its spinning bars, above and behind him, as he runs the wheel. At the end of this close-up, Jack's happy facial expression gives way to a look of dread, as he supposedly sees an approaching metal bar about to make contact with his forehead. This is impossible, since all bars are spinning behind him. After a close-up of the actual bar, in the next shot as Jack's forehead makes contact, he stands further back and is now at the center of the circular wood frame, so as to be in perfect alignment with those spinning bars. Plot hole: There are six metal bars mounted around the circumference of the water wheel's axle. In the wheel's interior, when Jack frees himself he lands on his feet, begins to run and promptly hits his head on one of those mounted axle bars, then falls out. Moments later, when Jack returns and runs in the wheel, he is shorter (way more than a foot) than all six of the mounted axle bars spinning with the water wheel, which would make that previous shot impossible, however humorous it is. Continuity mistake: On the water wheel, the characteristics (such as shape and size) and position of the hole Jack created keeps changing, once Jack is free of the wheel. (01:52:15)

Continuity mistake: After the second speedboat explodes, when Sean (as Castor Troy) pulls the speargun trigger, Castor (as Sean Archer) grabs the spear shaft before it can unload. As he grips the gun, the problem is that in some shots the slings are still loaded, but in other shots they are not. (02:09:00)

Continuity mistake: When Castor (as Sean) grips the speargun, in the following close-ups, the sling ends (with its wishbones) differ - just note the rings around the rubber ends.

Continuity mistake: When Castor Troy (as Sean Archer) says, "Every time you'll look in the mirror, you'll see my face," he proceeds to cut his face. The first cut he makes is straight down the side of his right cheek (viewer's left) and later that cut is curved and closer to his nose. This may appear to be a mistake at first glance; however, Castor's facial expression is different in the two images. When he made the cut, the cheeks were pulled away from the nose; in the second photo, the muscles are relaxed, moving the cut closer to the nose. (02:09:15)

Revealing mistake: Just as the red speedboat heads toward shore, before it crashes, the white and red truck cranes are on the dock and the white crane arm is angled backwards over the dock. In the shot facing the speedboat, as it explodes, that crane arm is now angled the opposite way, over the water, since it was used in the stunt.

Visible crew/equipment: When the red boat has hit the barrier and Castor Troy and Archer fly through the air, you can see a wire attached to the front of the boat just for a split second, then when it falls in the water and explodes.

Continuity mistake: After the incident with Karl, when Castor (as Sean Archer) and Jamie are in the living room, he opens the cigarette case and offers her one. The way he holds the case noticeably differs between shots.

Revealing mistake: At Erehwon, Sean (as Castor Troy) is given steel magnetic boots to wear, for the prison's important security feature. There is a close-up of these heavy duty boots during his 'exercise'. Then, when he is taken to the clinic there is another close-up, and there are a number of changes to the boots style. More importantly, one of the side "bolts" has fallen off, revealing that it was merely glued on the prop boots.

Continuity mistake: After leaving the Hummer, when Sean gets into the chopper the cabin interior changes in the following shots. This includes the windshield, hand grip, wall, etc.

Visible crew/equipment: At the Walsh Institute, after Sean gives his ring to Tito, when the stainless steel sliding door closes, the camera and a crew member, or likely the director, looking behind the camera lens is seen in the glass window reflection.

Continuity mistake: After Sean looks up his son's file on the computer, and types "case closed" on Castor's file, he rubs his head with his right hand in the close-up, but next shot he is rubbing his head with his left hand. (00:21:20)

Continuity mistake: After Sean types "case closed" on Castor's file, Dr. Miller walks into Sean's office and holds out Pollux's Zip Disk for Sean to take. In the next close-up, the Zip Disk has flipped over and faces the opposite way.
