Continuity mistake: When Bond and Natalia eject from the helicopter, you can clearly see two white parachutes. But when the capsule lands, in two scenes you see the parachutes being red and white. (01:12:20)
Continuity mistake: In the tank chase scene, when Orumov's car goes down the alley, the headlight and corner of the car are smashed. However, when the car emerges from the alley, the corner is intact. (01:21:08)
Continuity mistake: At the start of the movie where Bond is fighting the man on the runaway plane, Bond jumps out of the plane with the pilot (they hit the motorcycle). In mid-air, Bond has no gloves on, and when he gets back on the motorcycle, he has black leather gloves on. (00:09:08)
Trivia: This flub needs slow-mo to catch, but is worth listing as trivia. When Bond jumps off the dam, they do a shot following him along his descent down the ledge of the wall. In a shot only visible during three or four frames, but they catch a shot of the ledge before panning down in which you can see the film crew watching on top together with a van or two. (00:01:50)
Continuity mistake: In the scene when 007 is in Monaco, he takes a photo of the admiral and his female company. When he is calibrating the camera its to one eye. In the other shot he has the camera to the other eye. (00:21:55)
Continuity mistake: Near the beginning just before the radar station is destroyed you can see over Natalya's shoulder on the large map, that the Goldeneye is somewhere over northern Russia, the very next frame shows the map with the Goldeneye marker flashing because it is over its target in west Russia. Continuity mistake: When Bond is flying the plane (after freefalling from the motorcycle) over the chemical plant at the beginning of the film, the people who have been chasing him down the runway have vanished. (00:10:17)
Revealing mistake: Whilst in Q's workshop, Bond places his envelope on Q's X-Ray Document scanner. If you pay close attention to the screen (it's easy to notice anyway) his flight ticket has got VOID written on it 4 times... (00:51:54)
Continuity mistake: When James is chasing Natalia and Ouromov through St. Petersburg in the tank, James crashes through various walls and statues, but when he is looking down at Xenia, Ouromov, and Natalia as they get on the train, the tank is wiped clean with no sign of bricks, Perrier cans, or any other debris. Revealing mistake: Just before Bond and Natalya are ejected from the Tiger, a mid-range shot reveals a pair of dummies in their place. (01:12:15)
Continuity mistake: When Bond is running after the plane, we see 2 motorcycles in front of the group of soldiers. We then see the whole runway from a long shot and the motorcycles are not in sight. (00:08:20)
Continuity mistake: Xenia's hair is tied up in a pony tail, but when Bond fires at the helicopter it's loose. Then it goes back to a pony tail. (01:43:00 - 01:44:01)
Revealing mistake: When Xenia dies and the helicopter is about to explode, the pilot is replaced with a very obvious stiff dummy which falls forward without control.
Other mistake: When Xenia's helicopter crashes the pilot vanishes.
Revealing mistake: When Bond kicks Trevelyan off the antenna ledge, the side-on shot reveals both stuntmen instead of Pierce Brosnan and Sean Bean. (02:02:41)
Continuity mistake: When Bond throws his gun on the bench in the sauna, the barrel is facing away from the towel. When Bond goes to get it a few moments later and Ontopov attacks him, the barrel is now facing the towel. (01:02:20)
Continuity mistake: When Q goes to pick up the plane ticket off the X-ray document scanner, the envelope is perched on the lip of the tray. When the camera cuts to an extreme close up the envelope is flat on the tray. (00:49:40)
Continuity mistake: When Xenia is crushed to death in the crotch of a tree by her safety harness, her head is looking down. When Bond and Natalya look at the dead Xenia after the helicopter explosion, her head is now looking up. (01:40:00)
Continuity mistake: As Bond drives over the jeeps and police cars one of the flaps on the tanks gets damaged. It is then back in one piece in the next shot. This is not to be confused with when one flap goes missing. (01:20:50)
Continuity mistake: In the tank chase it is made very clear Bond is stood through the hatch while driving, However in some shots, he's not visible, but appears an instant later. (01:18:00)
Chosen answer: The real answer, which I believe was explained better in the game or in special features. Regardless, I did hear, is that Ourumov wasn't aiming directly at Alec's head, but to the side and shot the ground behind him. But made it look like he shot him in the head. Alec would have felt the bullet whiz by him.

It has been a while since I watched the movie, and my copy is a VHS... but I believe if you watch very closely you can actually see the bullet hit the ground behind him with a tile breaking. I don't remember for sure if you can, but I seem to remember they did add that little effect.
