Visible crew/equipment: During the final stand off between Decapitron and the Totem in Puppet Master V, you can see about three strings yank the Totem back, as he is blasted by Decapitron's electricity.
Revealing mistake: In several scenes, it is obvious that the animal actor playing Bagheera is actually a black jaguar. For example, the jaguar is larger and heavier than the leopard/panther. The scene where Bagheera tackles a poacher, he is much too large and heavy to be a leopard/panther.

Anyone who has been through Army Basic Training can tell you, a Drill Sergeant will NEVER let "Sir" or "Ma'am" pass without comment. Continuity mistake: In the sequence in which Marisa Tomei and Bonnie Hunt are getting ready to meet Damon Bradley, Marisa is sitting in front of the mirror fixing her makeup, then Bonnie looks in the mirror and is putting on her right earring, then in the next shot, she is grabbing the earring from the bureau and putting it on again in the right ear.
Plot hole: The reason given for the cop having psychic powers is that he has a well-developed hypothalamus - but that is the part of the brain responsible for homeostasis (maintaining a constant internal environment for the body). So even if you had a particularly well-developed one, you might sweat more when you get hot but how could it make you psychic?
Visible crew/equipment: You can see the strings on Pumpkinhead when they are killing him at the end.
Audio problem: When the good guys finally get the Chevy V8 running, the sound it makes sounds more like a moped than a high-performance engine. It has headers and no mufflers, so it should have been quite loud.
Audio problem: About halfway through the movie, Richard and Fantasy reunite on the island. Richard says, "Fantasy." and Fantasy responds with, "Well, what were you expecting honey? The tooth fairy?" When she says "the tooth fairy" her lips are saying something completely different (and it's even worse, since it's a close-up of her face).
Visible crew/equipment: When Capa talks to Mrs. Niedelmeyer, there's a 3-foot long shadow of the boom mic on the door.
Continuity mistake: When Kersey shoots Vasquez behind him we see the squibs under his shirt make the classic plus shaped holes in that shirt, but every time we Vasquez's body after that there are no holes in his clothes at all and the bloodstains also move between the time he's seen on the floor and the time he's unpacked from the crate he ends up in.
Visible crew/equipment: When Blankman and his brother escape from the bank just before it explodes they are diving on the ground to escape the explosion. You can see that Blankman's brother is a stunt double. (01:04:30)
Continuity mistake: When Meo and 16 are competing with weapons, 16 has a staff but when he bows to grand master he has 2 swords in his hand.
Factual error: Near the end of the film, the movie changes locations to Seattle. A skyline is shown and it is identified as Seattle. The city skyline that is shown is actually Philadelphia.
Continuity mistake: When Claire Laurent and Martine are being interviewed at the beginning of the movie about Reverend Bird's murder, Clair says to the reporter, "I have no comment, now let me by please". Later when they show the clip of the interview on the TV she says "I have no comment, now please let me by"
Continuity mistake: While Bill waits for his brothers in the car, he looks toward the store and watches his brothers inside. The first shot of Bill's perspective of looking at the store, there is a reggae black man walking by. Then it switches back to Bill's face, then again at the store and the man walks by the window a second time in the same direction. When Bill gets out of the car and crosses the street the black man walks by a third time.
Continuity mistake: In the credits, Cammy has been mispelt "Cammie"
Continuity mistake: During the last flashback, Spab is seen pulling out an Uzi that he had never acquired.
Visible crew/equipment: The shadow of a boom mike is visible above Mathilda's mom, when Mathilda begins to cry. (00:14:55)
