When we first see her talking next to her computer, this shot begins looking over a rail and pulls back. As the camera pulls back, the track that it is sitting on is visible on the bottom right of the screen. You can also see electrical wire next to the track.

Continuity mistake: After Creasy cuts off the first finger of the first man he kills, he is using the car cigarette lighter to stop the bleeding. When the shot changes to show him using the lighter, the man's finger is seen bent around the top of the steering wheel.

He knew from the first bullet to his body he was not going to make it out alive so he did what he had to do to get as far as he possibly could. He believed she was dead, he had nothing to live for, he was killing as many as he could before he fell off the wagon.
