Continuity mistake: During the dinner scene, when Frank and Joan almost kiss, they keep changing distance to each other. Every time the shot is from over his shoulder, their faces are really close together, almost touching. When the shot is from over her shoulder, they are further apart.

Continuity mistake: Near the end, when Howard is in Quentin's place moving furniture, when he gets to the last chair near the fireplace in the overhead shot he just moves it with his foot, but in the following closeup he has his left hand on the marble above the fireplace. (01:32:20)

Continuity mistake: When Howard's men bail the guy out who was with Bobby Saint at the arms deal, they are beating him up in Saint's club. When Quentin says his line after grabbing the guy by the hair he sits back in his chair. In the wide shot his cigarette is under his hand and is about to take a puff, but in the following shot the cigarette is nowhere near his mouth and it is now in his fingers. (00:09:00)

Continuity mistake: When Castle's father is making a toast at the family reunion, his martini class spills slightly towards Frank Castle, causing Frank to flinch and laugh at his dad's drunkenness. However, the shot changes to a different angle and Frank is sitting down as if he never moved.

Continuity mistake: When the cops turn up at the firearms deal, someone says, "Your under arrest," and there are two to three cops. When Frank says his line, the camera cuts to an overhead shot and there are now 6 cops all grouped together standing only a few feet from Bobby, Frank and the others. (00:04:00)

Continuity mistake: After the Punisher has shot Howard Saint, he tosses down his pictures of Quinton, proving that his wife was never unfaithful. When he first tosses the pictures, at least one lands a few inches beyond Saint's head, and the rest all apart. But in the next shot, they are all together, in front of his eyes (and all facing the same general direction, conveniently).

Continuity mistake: When Harry Heck is taking out his guitar from its case, watch as he puts his cigarette between his ring and pinkie fingers, then watch as it appears back in his mouth and then back between his fingers without time to make the switch. (01:05:00)

Continuity mistake: When the Punisher is pretending to burn the man with the torch and then lets him down, in one shot the popsicle stick is showing and on the later shots the top of the stick is no longer visible.

Continuity mistake: When Saint's son is on the boat full of money, and he hears a sound, he has a cigarette in his right hand, but in the following shot the cigarette is in his mouth. (01:04:40)

Factual error: In the scene where Micky and Howard are discussing Livia's parking ticket, Micky mentioned that date the ticket was issued was September 5th, a Thursday. The movie takes place in 2003 where September 5th was a Friday.

Continuity mistake: When the three tenants of the apartment where Frank is staying come out of their rooms to investigate the noise, Joan walks up to the window and before she gets to the window crosses her arms, but in the following shot she is crossing her arms again. (00:38:20)

Continuity mistake: When Frank's family are at a beach-house, while his dad is making a speech Frank is looking at his wife, but in the following shot he is looking at his dad standing up with his right arm up to his cheek, which wasn't there in the previous shot. (00:15:05)

Continuity mistake: When Maria is fleeing from Saint's men, the boat cracks the windshield of their 4x4, but in the following shot the windshield is completely intact. (00:25:30)

Continuity mistake: The bullet-hole scar on Frank's chest moves closer to, and further away from his sternum several times during the film.

Continuity mistake: At the end of the film when Frank Castle is standing over Howard, Frank grabs Saint's right leg (you can tell its his right leg by looking at the shoe), but in the following shot he is holding Saint by the left leg. (01:45:35)

Continuity mistake: In the lobby scene, before Frank Castle shoots two of Howard Saints men, he puts down his briefcase, and brushes his coat back, and in the following shot he brushes his coat back again. (00:49:00)

Revealing mistake: When the older Saint boy is pointing his gun at Frank's chest, you can see the squib under his shirt, you don't actually see him get shot until the flashback later in the movie, but you can see that the blood explodes from the same spot.

Audio problem: At his "pool party", Mr. Saint is talking to a group of men, and there is a female singer standing in the background, her mouth is moving as though she is singing, but there is only music from the band.

Continuity mistake: When Howard's men are beating up the guy in Howard's club, Howard walks in and points a gun at the guy on the floor. The guy is wearing a jacket which sits on his shoulders close to his neck, but in the following close-up the jacket has moved somewhat off of his shoulder exposing his top under the jacket. He has nothing to live for and is merely going through the motions of life, and coupled with his guilt over selling out Frank and getting his family killed, Frank concludes Jimmy needs to take his fate in his own hands.
